Immunizations

All students entering school are required by State law to have a South Carolina State Certificate of Immunization (DHEC 4024).  

South Carolina Department of Health and Environmental Control (SCDHEC) Immunization requirements for the 2023-2024 school year may be found here.

If any required immunizations are missing, the parent/guardian will receive a Certificate of Special Exemption (DHEC 1123) that allows the student to continue to attend school until the required immunizations can be received or their records can be transferred from another location. The exemption expires 30 days from the date issued. This form can only be issued to the student once.

If a SC Certificate of immunization has not been turned in to the school by the expiration date on the Certificate of Special Exemption (DHEC 1123), your student will not be allowed to attend school until they have received the required immunization(s) or exemption(s).

If you need your student's immunizations transferred onto a SC State Certificate of Immunization, please contact your medical provider or a York County Health Department.
